There are ischemic peripheral areas that never recover, despite repeated anti-VEGF injections (brown grass). The theory is with brown grass, (especially assuming a limited water supply), removing the grass (via targeted panretinal photocoagulation [PRP]) might decrease the amount of water that is diverted. These include chronic limb ischaemia, atrial fibrillation, recent MI (resulting in a mural thrombus), or a symptomatic AAA (ask about back/abdominal pain) and peripheral aneurysms.
